» Skilled Nursing
Our Registered Nurses are dedicated to promote health, prevent disease, and help our clients with their illness. Other responsibilities are listed below:
- Observe, assess and record signs and symptoms and progress in patient’s status.
- Conduct a physical examination of the client, including vital signs, physical assessment, mental status, and coordinates with primary physician.
- Develop and manage nursing care plans, advise patients and their families on proper care.
- Assess the client’s environment to determine what assistance will be available from the family members caring for the client.
- Evaluate the client’s condition and home situation to determine additional services that will be required and its frequency.
- Liaison between the client, the client’s family, and the client’s physician.
- Consult with the attending physician concerning the plan of treatment.
- Teach and instruct clients about effects and side effects of medication regimen, disease process and plan of treatment.
- Provide Skilled Nursing:
- Administer oral, parenteral and peripheral medications.
- TPN Administration
- Tracheostomy, Gastrostomy, Colostomy, Illeostomy and Nephrostomy care.
- Blood drawing.
- Urinary Catheterization.
- Glucose monitoring.
- Wound Care (assessment, measurement, treatment and dressing change).

» Physical Therapist
Our Physical Therapist provides treatment that improves joint mobility, relieve pain, and prevent or limit permanent physical disabilities of a client suffering from injuries or disease. Other responsibilities are listed below.
- Assess and evaluate clients by applying diagnostic and prognostic muscle, nerve, joint and functional test.
- Treats clients to relieve pain and develop or restore functions to maintain maximum performance.
- Document and coordinates with the primary physician the client’s reactions to treatments and any changes in the client’s condition.
- Instruct the client and the family regarding the total physical therapy program.
- Teach and instruct clients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, and wheel chair etc.).
- Makes arrangement for out-client services, which cannot be given in the home.
- Participates in development and periodic revision of the plan of treatment.
- Participates with staff, clients and physician in discharge planning activities and completes the physical therapy discharge summary.

» Speech Therapy
Our Speech Therapist evaluates and provides speech therapy services to clients in the home as prescribed by the attending physician. Other responsibilities are listed below:
- Evaluate client’s speech and language abilities, both defects and assets and perform periodic re-evaluations.
- Plans and provide rehabilitative services for speech and language disorder
- Document type of treatment and client’s reactions to therapy provided on the progress notes.
- Selects and administers diagnostic and therapeutic techniques and materials.
- Instruct and counsels other health team personnel and family members in assisting clients in improving, correcting and accepting his/her disabilities.
- Participates in the development and periodic revision of the physician’s plan of treatment.
- Coordinates with staff, client and physician in discharge activities and complete speech therapy discharge summary.

» Occupational Therapy
Our Occupational Therapist works with clients who have physical disabilities or who suffer from chronic medical conditions. Other responsibilities are listed below:
- Evaluate and establish a goal based on the client’s level of functioning and movement and performs re-evaluation as needed.
- Counsel and recommend the client prescribed therapeutic self-care, and creative activities that are directed toward improving independence and physical and mental functions.
- Provide periodic written progress reports of the client’s progress in therapy to the physician.
- Inform the family of total occupational therapy program.
- Coordinates with other health team personnel in the care of clients regarding occupational therapy.
- Train clients and their families in the proper use of prosthetic, orthotic and assistive devices (canes, walkers, wheelchairs, etc.).

» Medical Social Worker
Our Medical Social Worker helps our clients to improve their lives. Other responsibilities are listed below:
- Provides rehabilitative and supportive casework geared to restoring clients to their optimum level of social and health adjustment. This includes assistant clients and their families to understand, accept and follow medical recommendations.
- Helps clients utilize the resources of their families and the community. This may be accomplished by either referring the clients to resources or acting as an intermediary on behalf of the clients in their dealings with other health and welfare agencies.
- Assist clients and their families with personal and environmental difficulties, which predispose them toward illness or help out with obtaining maximum benefits from medical care. These range from counseling members of the client’s family to assisting clients with admission to a nursing home.
- Consults with the physician and other members of the health team for the purpose of assisting them to understand the significance of social, emotional and environmental aspects related to the client’s health problems.
- Participates in discharge and in-service programs; complete the MSW discharge within the framework of the Agency policy.

» Home Health Aide
Our Home Health Aide provides personal care and related services in the home. He/She functions under the direction, instruction and supervision of the Registered Nurse. Other responsibilities are listed below:
- Provides assistance to clients in personal care to maintain good personal hygiene (bathing, dressing, oral care, skin care, foot and nail care).
- Assist clients with ambulation, transfers, use of assistive devices (cane, walker, wheelchair, etc).
- Assist clients with dietary needs such as food planning and preparation.
- Assist clients with bowel and bladder elimination.
- Assist in maintaining a healthful and safe environment.
- Assist the therapy personnel as needed with rehabilitative process.
- Document all task performed during her/his visit and coordinate any changes with client’s mental or physical condition to the Registered Nurse or Supervising nurse.
